Please Have A Seat And Fill Out This Form analyzes what it means to be sexually active in today’s “liberated” society, while speaking about Davenport's experiences and the burden that her past decisions have put on the present. Davenport has mimicked a waiting room environment in order to inflict a level of discomfort for viewers just as she, and many others, experience when walking into a clinic. Viewers are asked to have a seat and fill out a medical history form. As a women that goes to Planned Parenthood for annual exams, birth control and STD testing, the installation is a critique of the stigma around health care providers like Planned Parenthood.
